Background of the Study
Hospital-acquired infections (HAIs) are a major public health concern worldwide, contributing to increased morbidity, mortality, and healthcare costs. These infections, also known as nosocomial infections, occur in healthcare settings and are often associated with inadequate infection prevention and control measures, particularly poor hand hygiene practices among healthcare workers (World Health Organization [WHO], 2024). Hand hygiene is widely recognized as the most effective measure to prevent the transmission of infectious agents in healthcare settings. Despite numerous awareness campaigns and policy initiatives promoting hand hygiene, compliance among healthcare workers remains suboptimal in many developing countries, including Nigeria (Ogunleye et al., 2024).
Zamfara State, located in northwestern Nigeria, faces significant healthcare challenges, including inadequate healthcare infrastructure, limited access to clean water, and overcrowded hospital conditions, all of which contribute to the spread of HAIs. Studies have shown that the burden of HAIs is particularly high in resource-limited settings, where compliance with infection control protocols is often poor (Adebayo et al., 2023). The transmission of pathogens such as Staphylococcus aureus, Klebsiella pneumoniae, and Escherichia coli in hospital environments underscores the critical need for improved hand hygiene practices among healthcare workers (Ahmed et al., 2024).
Although WHO and the Centers for Disease Control and Prevention (CDC) recommend the use of alcohol-based hand sanitizers and regular handwashing with soap and water, studies have found that many healthcare workers in Nigeria do not consistently adhere to these guidelines. Factors contributing to poor hand hygiene compliance include lack of awareness, inadequate supply of hand hygiene materials, high patient loads, and cultural perceptions of cleanliness (Salako et al., 2023). Given the growing concerns about HAIs in Zamfara State, this study seeks to assess hand hygiene practices among healthcare workers and their impact on the prevalence of HAIs, with the aim of providing evidence-based recommendations for improving infection control in hospitals.
Statement of the Problem
Hospital-acquired infections remain a significant cause of morbidity and mortality in Nigeria, and Zamfara State is no exception. Despite global and national initiatives emphasizing the importance of hand hygiene, healthcare workers in many hospitals fail to adhere to recommended hand hygiene protocols. This lack of compliance has been linked to the rising incidence of HAIs, which not only prolong hospital stays but also increase healthcare costs and the risk of antimicrobial resistance (WHO, 2024).
In Zamfara State, anecdotal reports from healthcare professionals suggest that HAIs are prevalent, particularly in tertiary and secondary healthcare facilities. However, there is a lack of empirical data on the extent of the problem and the specific role of hand hygiene practices in controlling these infections. Furthermore, existing research on infection control in Nigeria has largely focused on urban centers, leaving gaps in understanding the situation in rural and semi-urban hospitals, where resource constraints are more severe.
Without concrete data on hand hygiene compliance and its impact on HAIs in Zamfara State, healthcare institutions cannot effectively implement targeted interventions to reduce infection rates. This study aims to fill this gap by assessing hand hygiene practices among healthcare workers and evaluating their impact on the prevalence of HAIs in Zamfara State. The findings will contribute to policy recommendations and the design of training programs to improve infection prevention and control measures in hospitals.

1. To assess the level of hand hygiene compliance among healthcare workers in selected hospitals in Zamfara State.
2. To evaluate the relationship between hand hygiene practices and the prevalence of hospital-acquired infections in Zamfara State.
3. To identify challenges and barriers to effective hand hygiene practices in healthcare facilities in Zamfara State.

1. What is the level of hand hygiene compliance among healthcare workers in Zamfara State?

Research Hypotheses
1. There is a significant correlation between poor hand hygiene practices and the prevalence of hospital-acquired infections in Zamfara State.
2. Improved hand hygiene compliance among healthcare workers will lead to a reduction in hospital-acquired infections.
3. Lack of adequate water supply and hand hygiene materials significantly affects compliance with hand hygiene protocols in Zamfara State.
Scope and Limitations of the Study
This study focuses on assessing hand hygiene practices among healthcare workers in selected hospitals in Zamfara State and their impact on the prevalence of hospital-acquired infections. The study will involve hospitals from urban, semi-urban, and rural areas to provide a comprehensive analysis. The primary limitation of this study is the reliance on self-reported hand hygiene compliance, which may introduce response bias. Additionally, resource constraints may limit the ability to conduct laboratory-based confirmation of infection sources.

• Hand Hygiene: The practice of handwashing with soap and water or using an alcohol-based hand sanitizer to prevent the spread of infections (WHO, 2024).
• Hospital-Acquired Infections (HAIs): Infections that patients acquire during their stay in a healthcare facility that were not present at the time of admission (CDC, 2023).
• Compliance: The degree to which healthcare workers adhere to recommended hand hygiene protocols (Ogunleye et al., 2024).
